In order for patients to qualify for the NJ Medical Marijuana Program, a physician must certify that a patient has one or more of the following qualifying conditions:

Amyotrophic Lateral Sclerosis (ALS)

Cancer

Muscular Dystrophy

Multiple Sclerosis

Inflammatory Bowel Disease (IBD)/Crohn’s Disease

Terminal Illness (physician determined less than 1 year prognosis)

Tourette’s Syndrome

Chronic Pain due to Musculoskeletal Disorders and Chronic Visceral Pain

Anxiety

Migraines

These conditions apply if the patient is resistant or intolerant to conventional therapy:

 

Glaucoma

Intractable Skeletal/Muscular Spasticity (ISS)

Post-Traumatic Stress Disorder (PTSD)

Seizures and Epilepsy

This last set of conditions apply if severe or chronic pain, severe nausea or vomiting, cachexia or wasting syndrome results from a condition or treatment of:

 

Cancer

Human Immunodeficiency Virus or Acquired Immune Deficiency Syndrome (HIV/AIDS)

Required Documents

Document 1 - Government issued photo identification – Required one of the following:

 

  • Current NJ digital license

  • Current NJ digital non-driver ID card

  • NJ County ID Card

Document 2 - Proof of current New Jersey residency - Required (P.O. Boxes NOT Accepted)

  • one of the following:

  • Utility bill issued in the past 90 days that shows your name at your current address

  • Utility Bills accepted: Gas-Electric-Water-Sewer-Cell Phone-Cable (Television/Internet/Phone)

  • Any correspondence from IRS or NJ State tax office within the last year

Document 3 – Proof of government/military assistance - Optional *

  • one of the following:

  • NJ Medicaid

  • Food Stamp Benefits

  • NJ Temporary Disability Insurance benefits

  • Supplemental Security Income (SSI) benefits

  • Social Security Disability (SSD) benefits

  • Military Discharge Papers (DD214)

+ ALL NEW PATIENTS ARE REQUIRED TO BRING IN THEIR MEDICAL RECORDS TO PROVE THEIR QUALIFYING CONDITION
